Found a defect in a Next Level Apparel garment — fabric flaw, dye migration, shrinkage, color shading, or a shipped quantity that doesn't match your order? File a claim below and our quality team will review it.

Before you file
- One form per style. If your claim covers more than one style number, submit a separate form for each, this keeps the size breakdown and decoration details accurate per style.
- Photos are required. Attach clear photos of every claimed defect. Forms submitted without photos will be delayed while we follow up for them.
- Have your order details on hand: PO number, order number, order date, style number, and color.
